Myelomegakaryoblastic transformation of essential thrombocythemia.

چکیده
Essential thrombocytherrlia, considered one of the least common of the myeloproliferative disorders, is being diag nosed more frequently with the increasing use of automated blood cell counters. Essential thrombocythemia described by Epstein and Goendel in 1934 is a diagnosis of exclusion i.e., exclusion of all sy stemic conditions associated with thrombocytosis in an individual with a persistently elevated platelet count of more than 600x10/L (600,000/ul). Prognosis of essential thrombocytherrlia is excellent, with a median survival in excess of 10 years. However, 10 percent of cases undergo blastic transformation. A case of essential thrombocythemia presenting in myelomegakaryoblastic transfomlation is reported, due to its rare occurence and presentation.

INFERIOR VENA CAVA THROMBOSIS IN A PATIENT WITH ESSENTIAL THROMBOCYTHEMIA
Essential thrombocythemia is a chronic myeloproliferative disorder characterized by a sustained proliferation of megakaryocytes, which leads to increased numbers of circulating platelets. Hemorrhagic and/or thrombotic episodes are frequent, and thrombosis of both veins and arteries may develop. Vessels in unusual sites may be involved, e.g., the hepatic veins, mesenteric veins, and the dig...
متن کاملJAK2 or CALR mutation status defines subtypes of essential thrombocythemia with substantially different clinical course and outcomes.
Patients with essential thrombocythemia may carry JAK2 (V617F), an MPL substitution, or a calreticulin gene (CALR) mutation. We studied biologic and clinical features of essential thrombocythemia according to JAK2 or CALR mutation status and in relation to those of polycythemia vera. The mutant allele burden was lower in JAK2-mutated than in CALR-mutated essential thrombocythemia. Patients with...
متن کاملLeukocytosis at Diagnosis in Patients with Essential Thrombocythemia Is a Risk Factor for Transformation into Myelofibrosis
Myelofibrosis (MF) represents the major long-term complication of essential thrombocythemia (ET). There is evidence that leukocytosis at diagnosis is associated with poorer survival in patients with ET. In this study, we retrospectively evaluated 143 patients with ET, diagnosed in agreement with WHO criteria, followed in a single centre over 10 years. Nine of them transformed into MF (post-esse...
متن کاملDiagnosis and treatment of thrombocythemia in myeloproliferative disorders.
Myeloproliferative disorders originate in the clonal expansion of a transformed pluripotential hematopoietic progenitor cell. This results in a group of syndromes that include polycythemia vera, essential thrombocythemia, chronic myelocytic leukemia, and agnogenic myeloid metaplasia. Diagnostic criteria forpolycythemia vera and essential thrombocythemia were codified by the Polycythemia Vera St...
متن کاملManagement of thrombocythemia
Essential thrombocythemia is a clonal myeloproliferative neoplasm characterized by an elevated platelet count, the potential for both microvascular and macrovascular sequelae, and a risk for transformation to myelofibrosis or acute myeloid leukemia. A systematic and detailed initial analysis is essential for accurate diagnosis of essential thrombocythemia, as many etiologies are reactive and be...
